www.BerdonClaimsLLC.com/unitedhealthcare is the web page where you can receive a copy of the claims information from United HealthCare Class Action Litigation. You first need to submit a claim infomration form request on berdonclaimsllc.com/unitedhealthcare whether you are a subscriber or provider.  In order to receive a copy of the claims information that was made available by the Defendants of the case for covered out of network services or supplies from Jan 1 2002 to May 28 2010, you need to fill in a form.  This will allow and help you to file a Group B or Group C claim.

to fill the form, you will need to provide your subscriber personal information including your insurance policy ID number and social security number. Like others, Milton Memorial Birthing Center(MMBC) submitted a Proof of Claim Form and was told that we did not need to do anything additional. MMBC submitted these forms on August 30, 2010. re the united health care litigation. both my husband and myself have our own policies (family). we submitted our claim forms at the same time. i have signed return receipts for both. my husband received his check around 3/2/12.
