www.JDPower.com/Survey/Amfam is the 2010 American Family Insurance Satisfaction Survey webpage.  This website  features a text box where users can input their pin to complete the survey. The purpose of the survey is to collect feedback and opinions from the customers. The pin number appears to be sent to users who are client of the company.  This is stated very clearly in the page and hence making it very comprehensive. There is an easy to spot direct link to American Family Insurance's main site, which is very convenient for the first time user.

In addition to this, at the bottom of the page is located a link to an email from which the user may choose to opt out of future contact and/or surveys with American Family Insurance. The website .jdpower.com/survey/amfam states that the contact made to users is at random and that one may choose to end contact about such surveys at any point in time.
